Community Nursing Care: Our nurses work to prevent and manage illness and promote health across the lifespan by identifying barriers to healthy lifestyles and general wellness. We work with families and communities to empower individuals accessing care to promote healthy lifestyles and provide post-acute care to people in their homes.
- Our highly skilled team of nursing staff comprising of registered nurses, enrolled nurses and assistant nurses are adequately trained to offer:
- Overnight Nursing Care
- Either Active or inactive overnight supports.
- Chronic disease management
- Our qualified nursing staff can assist with managing your chronic disease management plan be it from you GP or other health practioners.
- Continence management
- If you suffer from incontinence, we can help you find ways to improve your lifestyle with a range of services delivered in the comfort of your own home.
- Wound and Stoma Care
- Do you have a wound, ulcer, tracheostomy, or stoma and want to continue living in your own home, we can visit you at home to provide the nursing care you need.


- Diabetes management
- We will work with your GP or specialist on your care and other Allied health services including dietetics, physiotherapy or podiatry.
- Medication preparation & administration
- Our qualified nurses can support participants to develop and maintain a personalised care plan
- Palliative Care
- We offer support in life-limiting illnesses in all stages be it stable, unstable, deteriorating and terminal and covers.
- Dementia Management
- Our staff genuinely care about our participants and will and spend time getting to know you; your history, your family, your likes and dislikes, to ensure we’re delivering the best care possible. Our services for people living with Dementia include personal care, household chores/assistance, transportation, community nursing, community interaction.
